The Form EVTA-1, Program Plan Application provides applicants with a pre-formatted document for developing a program plan. The EVTA-1 Application is available in both Microsoft Excel and Adobe PDF formats. The Excel version of the application performs calculations of certain fields and carries over values to other fields within the spreadsheet. Using the PDF version will require you to separately perform calculations and enter resultant values in the document and to manually carry over values based on line-by-line instructions. The Form EVTA-1 Program Plan Procedures and Instructions document is available in both Microsoft Word and Adobe PDF formats.

Global's, American Express' and Discover's customized New York State card acceptance guides are available for download as PDF documents. These guides provide agencies that are implementing credit card programs with step-by-step procedures to follow when accepting MasterCard, Visa, American Express and Discover branded cards.  These guides also cover such topics as how to prevent fraud, how to avoid chargebacks and how to process chargeback retrieval requests.
