You may find it helpful to see examples of the:

• notification letter
• pre-printed application (your unique STAR code that easily identifies your property will be on your application)

You need a few simple pieces of information to complete your application. You'll need:

• the STAR code for your property from your pre-printed application
• names and social security numbers for all resident property owners and their spouses
• to verify that everyone filed a New York State income tax return for 2006. (If anyone was not a New York State resident in 2006 and was not required to file a 2006 New York State income tax return, you will need the front page of your 2006 Federal income tax return.)

If you received more than one notification letter and pre-printed application, please review the information on the application carefully. You should only apply for a rebate for a home that you owned on the date provided on the application that was your primary residence.

If you believe you are eligible for the program and you have not received a rebate check, notification letter, or correspondence from us by the first week of November, please check back for further instructions.
